New Revision Rhinoplasty Staging System Published

141223-fps-265x265Throughout his career, Russell Kridel, MD, has made numerous significant contributions to his specialty of facial plastic surgery.  For example, he has pioneered several Rhinoplasty and Revision Rhinoplasty surgical techniques that are considered the gold standard among peers and taught in medical schools throughout the U.S. and the world.

His passion for advancing medical techniques and sharing that knowledge with other physicians, along with the ultimate goal of advancing the quality of patient care and surgical success, is once again revealed in his recent journal article, “A Staging System for Revision Rhinoplasty.”

Dr. Russell Kridel and Dr. Regina Rodman, recently introduced a new staging system, the PGS System, to determine the overall difficulty of surgery for revision rhinoplasty. Their findings have been released and published in the prestigious JAMA Facial Plastic Surgery Journal; “A Staging System for Revision Rhinoplasty.”

The 3 major components of the PGS system can be described as; the “P” represents “problem,” consisting of the specific anatomic anomaly with which the patient presents. The second component in our system is “G” for “graft,” based on the number of grafts required. The third component of this system is “S,” for “number of previous surgical procedures.” In addition, Kridel and Rodman have included a category “E,” for “patient expectations,” which is added after the stage of the patient’s condition has been determined through the PGS classification.

As Dr. Kridel states in the journal article, a standardized evaluation system such as PGS System, will allow meaningful comparisons of surgical techniques and evaluations of outcomes of rhinoplasty procedures among physicians.  In addition, the PGS preoperative staging system also help patients understand the degree of difficulty of their required repair and help manage expectations.

Russell Kridel, MD was a featured faculty member at the recent 14th International Course on Facial Plastic Surgery & Rhinology in Bogota, Colombia. As an internationally recognized leader on Rhinoplasty and Facelift surgical techniques, Dr. Kridel was invited to share his expertise with other surgeons attending the meeting.

Dr. Kridel was invited to discuss several surgical techniques he pioneered to an audience of multi-specialty surgeons attending the course from around the world. Facelift After Weight Loss

Losing massive amounts of weight is a wonderful achievement.  It takes a great deal of courage, will power and perseverance.  However, patients who have lost substantial amounts of weight are faced with significant sagging skin. Their skin has stretched so far that it doesn’t rebound as quickly or fully.

The patients’ new shape is sidelined by sagging skin and tissue that makes them feel unattractive. So, it’s not surprising that neck lift and facelift after weight loss requests have gone up proportionally with the increasing popularity of surgical weight loss treatments. In fact, a survey by the American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS) reported a 13 percent increase in the number of patients seeking facelifts and neck lifts to remove the excess skin and tissue.

This is not an instance where one stop shopping for a surgeon is the right thing to do.  Quite the contrary. It is critical that patients select a facial plastic surgeon to address sagging skin in the face and neck area.  Facial plastic surgeons specialize in cosmetic surgery from the neck up and have had years of extra training, including fellowship, specifically for this area of specialty.  Along the same lines, it is important to select a general plastic surgeon who is a body specialist to address the sagging skin that occurs following weight loss below the neck. But, the face is a far different and more complex anatomical area.  So, a body specialist does not necessarily make a good facelift surgeon.

While we all look at ourselves in the mirror every day, with Selfies, many people also check their looks on their mobile phones multiple times a day.In many ways, the growing trends of placing Selfies on social media platforms like Snapchat, Instagram and Tumblr, create greater demand for people to look the best they possibly can. Because these images are presented to the vast online world, people set a high standard for themselves of how they should look.

 

Rhinoplasty Surgery Requests Have Increased With Selfie Culture

The Selfie culture has shifted the focus of our interactions to a very visual level. Suddenly, facial flaws seem magnified and the need for plastic surgery procedures, like nasal surgery, known as Rhinoplasty, becomes more apparent. Your nose is a dominant feature of your face and any flaws that may concern you feel magnified on social media particularly, profile or three-quarter views. Increasingly, People seek Rhinoplasty surgery, commonly referred to as a nose job, to resolve many of their unwanted nasal features. In a March, 2014 survey by the American Academy of Facial Plastic & Reconstructive Surgery (AAFPRS), they found that the Selfie trend increases demand for facial plastic surgery, particularly for Rhinoplasty surgery. Many people, primarily those between 18 and 34, are looking for an edge to create a picture-perfect face and share it with the world. For example, shaving down a nasal bump or straightening a twisted nose can make the face look more symmetric from all directions.

“Imaged-based social media forces patients to look at their image with a more critical eye.  They want to make sure they look their very best to the online world, particularly to friends and loved ones, current or potential.  So it is completely understandable why we are seeing more younger patients seeking cosmetic procedures,” stated Houston facial plastic surgeon Dr. Russell Kridel.

If looking at your collection of selfies uncovers features you have always disliked about yourself, your reason to have Rhinoplasty surgery is no less valid than someone who broke their nose multiple times. The intensity of your dislike might be increased due to the amount of photos you are faced with daily, but it doesn’t change the fact that you don’t like your nose.

Post-Op Rhinoplasty Tips: Key Factors for Healing

At FPSA, follow-up care is one of the hallmarks that sets us apart from others.  Dr. Kridel and Dr. Sturm are fastidious and dedicated to the highest quality of care.

At your pre-op visit you will be given numerous post-op appointments.  It is important that you keep all of your follow-up appointments.

Here are some post op Rhinoplasty tips that can help your healing process following surgery:

 

 

 

  • Don’t take medicines containing aspirin or ibuprofen
  • Avoid bending or heavy lifting
  • Be careful not to bump your nose and avoid touching any of the dressings
  • Keep from blowing your nose until you are approved to do so
  • Try to sneeze or cough with your mouth open to reduce pressure inside your nose
  • Avoid wearing eyeglasses or sunglasses.  If needed, they can be taped above the nose
  • Protect your nose from the sun and use sunscreen
Our patients can contact us at any time.  If you have sudden increase in pain, fever, heavy bleeding or other unusual symptoms, you should call us immediately.

Beware of Unproven & Unscientific Plastic Surgery Procedures

We often get calls about various procedures that have been heavily advertised directly to consumers, including the following procedures noted in this article in Allure magazine: Vampire Lift (injecting patient’s own blood plasma), SmartLipo (Laser Liposuction), Stem-cell Lift and Artefill (for lips).  Unfortunately, the majority of these procedures do not have scientific evidence of effectiveness and have not been written up in peer-reviewed medical journals by widely respected facial plastic surgeons.  Sadly, we actually see patients that have tried some of these “fad” procedures and now require revisions by Dr. Kridel.

The Allure magazine article, titled “The 10 most overrated trends in cosmetic surgery”, provides an interesting overview of many procedures that Dr. Kridel and Dr. Angela Sturm-O’Brien do not perform or recommend with one exception — Ulthera. In the article the criticism about Ulthera was based on patient discomfort (and not about poor results). Since this article was published  in mid-2012, Ulthera received FDA clearance to non-invasively lift lax tissue on the neck and under the chin, in addition to the FDA clearance they already had for non-invasive eyebrow lift. Ulthera also completed a system upgrade — Ulthera Amplify — to increase effectiveness and reduce patient discomfort.  At FPSA we have the new Ulthera Amplify system and Dr. Russell Kridel and Dr. Angela Sturm-O’Brien have  found that the majority of patients do not require medication for treatment.  However, for even greater comfort during Ultherapy, we can also apply topical cream to the treatment areas.

Internationally recognized facial plastic surgeon, Dr. Russell Kridel spoke to the Houston Business Journal about the important issue of rising health care costs in the January 11, 2013 issue of the Houston Business Journal.

As the new President of the Harris County Medical Society for 2013, Dr. Kridel shared his thoughts on the implications of the Affordable Care Act for healthcare providers.

He also stated that one of his goals during his tenure is to focus on the obesity epidemic  — helping doctors counsel their patients on healthy lifestyles and provide to tools to help them empower their patients to take greater control over their health.
